Referral: Parents/Guardians are invited to refer their child to the 504 process if they believe that their child has a physical or mental impairment that could make their child eligible for Section 504 protections. Other individuals may also make referrals, such as teachers, school staff members, and health care professionals.
Eligibility: Once a referral has been received by the school, the campus Section 504 Lead will invite the parent/guardian to attend a Section 504 eligibility determination meeting where all supporting data will be reviewed by the committee. Based on the information presented and reviewed by the committee a determination will be made whether or not the student is eligible for Section 504.
504 Plan Development: Once a student is determined to be eligible for Section 504 protections, the Section 504 committee will develop a Section 504 plan that will outline the accommodations and services that the student needs in order to access the general education curriculum at school.
Monitoring: It is the district’s practice that the Section 504 committee will meet annually to review a student’s Section 504 plan to determine whether any changes are needed. Parents/guardians can also request a meeting if they believe that the Section 504 Plan needs to be reviewed or updated.
